To facilitate seamless care transitions, the App accesses third-party content and clinical data from referring Epic systems. This access includes gathering essential patient profiles, historical background details, and attached clinical documentation. Clinware Connect™ strictly accesses, processes, and transmits only the data that is explicitly authorized by the referring healthcare system to successfully coordinate patient placement and continuum of care.
2. Pricing and Fee Disclosure
Disclosure: Clinware Connect™ is free of charge for hospitals and referring health systems. There are no fees required for hospitals to utilize the app to send referrals, track patient placements, and coordinate care.
(Note: Post-acute and community care providers receiving the referrals participate under separate commercial agreements. Referring hospital systems incur zero costs for utilizing the application.)
